The Safeguarding Adult Concern and Notification form for sharing concerns about any adult with care and support needs with Adult Social Care has recently been revised. The revised form:
- is shorter
- is more user friendly, and
- now only asks for information that is likely to be known at the referral stage.

If you have saved any previous versions of the form, please delete these so that you only use the new form from now on.
